WHO WE ARE LOOKING FOR

The Community Health Worker is responsible for acting as bridge between health care providers, community services providers, and individual in the community to promote health, reduce disparities, and improve services delivery.  This person is a trained and trusted public health worker who is respected by the people they serve and applies their own unique understanding of the experience, socio-economic needs, language, and/or culture of the communities served. It needs someone organized enough to handle all the details. WHAT YOU WILL DO
  • Work with Public Health Specialists, Health Educators, Peer Navigators, Primary Care staff and community agencies to link patients to needed preventative health services to prevent illness, improve skills for self-management of chronic conditions, and promote healthy lifestyle behavior change.
  • Spend significant time in the community, in homes, at gathering places, in primary care practices, and other agencies doing outreach to build relationships with adults in Marion and Johnson Counties.
  • Provide health education to community members regarding the Medical Home, Behavioral Health Services, Emergency Department, Express Care Clinics, and on how to use these existing systems of care.
  • Refer and link to a medical home for preventive health care, age appropriate cancer screenings, self-management education for diabetes, chronic pain, and other chronic conditions.
  • Link individuals to community resources. 
WHAT IT TAKES TO SUCCEED
  • Computer skills, including Microsoft Word, Excel, Outlook.
  • Work independently with minimal supervision.
  • Accept personal accountability for deliverables and actions.
  • Actively listen, effectively communicate verbally and in writing as appropriate for the needs of the patients.
  • Adaptable to change.
  • Maintain composure in difficult situations
  • Use of electronic health records.
  • Demonstrates knowledge of performance improvement methods.
  • Knowledge of processes, tools, techniques, and practices for assuring adherence to standards associated with accessing, altering, and protecting organizational data.
  • Universal precautions.
  • Develop relationships with patients, hospital departments, and other external parties.
  • Make decisions and use judgement in ambiguous situations.
