  • Ensure optimal document image results by making minor maintenance and system setting adjustments to the scanning equipment as required and validates the integrity of medical record data by ensuring that documents are clear, legible and labeled with appropriate patient identifiers.
  • Perform technical troubleshooting with scanning equipment and communicate issues to the IS CONNECT system.
  • Determine document types within the document imaging system to ensure the accuracy of the medical record.
  • Identify and recommend opportunities for workflow improvement, procedures, and policies for prepping, scanning, indexing, and analysis techniques.
  • Analyze the electronic medical record in accordance with regulatory requirements and medical staff Rules and Regulations/Bylaws, assigns deficiencies in the EMR for signatures and missing documentation to ensure timely, accurate completion and integrity of the medical record.
  • Answer calls, serve walk-in Release of Information requests.
  • Coordinate delinquent record suspension process through communication with Medical Staff leadership and delinquent providers.
  • Demonstrate technical expertise of the electronic health record, document imaging application, and scanning equipment.
  • Evaluate and assess declined deficiencies and appropriately re-assign to the provider based on a review of the medical record.
  • Identify and escalate documentation completion concerns to HIM leadership.
  • Monitor assigned work queues and other system functions as needed.
  • Perform quality review of the medical record upon deficiency completion to ensure data elements and documentation requirements are satisfied.
  • Perform technical troubleshooting and reports system and/or equipment issues through the helpdesk system.
  • Provides ongoing feedback to clinicians regarding missing documentation and trends related to timely, accurate deficiency completion.
  • Understand the importance of clinician licensure scope of practice and health documentation requirements as they apply to medical record analysis.
  • Basic knowledge of computer office equipment.
  • Prior experience in a Customer Service role.
  • Epic EMR/Document Imaging experience.
  • Knowledge of types of health information and the rules and regulations surrounding their use.
  • Basic knowledge of release of information.
  • Chart Prepping for Scanning.
  • Critically evaluate and analyze information in written materials.
  • Modify behavior based on feedback or self-analysis of past mistakes.
  • Adapt and respond quickly to changing priorities.
  • Customer service skills.
  • Accept personal accountability for deliverables and actions.
  • Effectively communicate verbally and in writing as appropriate for the needs of the audience.



Minimum of 2 years experience, preferably working in an HIM/medical records department. Experience with EMR (Epic preferred), document imaging systems, and customer service strongly preferred.
