WHO WE ARE LOOKING FOR

There are times in health care when multiple resources have to work together: a patient, his or her family, foster team, primary care provider, case workers and more. In these situations, one person who can support each of those person’s needs is critical. The Clinical Case Coordinator listens and keeps care plans moving forward – and does so with compassion and skill. It’s not an easy job, but the rewarding ones rarely are.

 

Are you able to support a wide variety of people? Can you facilitate assessments, interventions and counseling needs? Are you detailed and able to document plans to keep the entire team on the same page? If you have these special skills, we have the perfect place to use them. 

WHAT YOU WILL DO

  • Develop individualized psychosocial plan of care with beneficiary and family and foster team approach by working collaboratively with beneficiary, family, primary care provider (PCP) and other members of healthcare team.
  • Perform appropriate interventions and document to meet needs of beneficiary, family and healthcare.
  • Provide education, make prompt referrals, and facilitate coordination of care.
  • Comply with all required documentation for medical/legal reporting situations.
  • Collaborate with health care team and evaluates treatment plan and interventions.
  • Excellent communication skills.
  • Identify psychosocial needs.

3-5 Years Experience

License(s): Licensed Social Worker (BSW)

Certification(s):

Travel: Up to 20%
