WHO WE ARE LOOKING FOR

There are times in health care when multiple resources have to work together: a patient, his or her family, foster team, primary care provider, case workers and more. In these situations, one person who can support each of those person’s needs is critical. The Supervisor of Clinical Case Coordination listens and keeps care plans moving forward – and does so with compassion and skill. It’s not an easy job, but the rewarding ones rarely are.

Can you supervise, facilitate and coordinate others? Are you able to support a wide variety of people? Are you detailed and able to document plans to keep the entire team on the same page? If you have these special skills, we have the perfect place to use them.

WHAT YOU WILL DO

  • Provide input into preparation, management and control of the Integrated Case Management budget.
  • Plan for and manage equipment, supplies and resources to meet department needs.
  • Direct department functions with appropriate staffing in place to meet the needs of the public served.
  • Develop, implement, monitor and analyze variances of a department performance improvement plan.
  • Oversee performance standards that ensure optimal service to the public.
  • Understand use of Milliman/ Interqual and other appropriate criteria.
  • Maintain organized data collection system, revises all appropriate cases in a timely manner.
  • Evaluate patient charts for medical necessity.
  • Conduct a thorough and objective evaluation of a patient’s current status including physical as well as psychosocial services as a patient advocate
  • Document all clinical information on patients on the day of admission and as needed.
  • Perform case management activities including assessment, planning, implementation, coordination, monitoring and evaluating.
  • Maintain awareness of the current continuity of care process, available resources and discharge planning/ case management relationship with precertification authorization and concurrent review.
  • Participate in case conferences as needed.
  • Proactively assess patient’s physical/ psycho/ social and age specific discharge needs.
  • Comply with all required documentation for medical and legal reporting situations.

WHAT IT TAKES TO SUCCEED

  • Supervise a team of professionals in an effective, confidential, and positive manner.
  • Experience with hospital information systems.
  • Experience in changing healthcare processes, such as Quality Improvement Team participation.
  • Strong organizational and leadership skills.
  • Excellent communication skills with medical staff.
  • Knowledge of community programs, services, and resources.
