WHO WE ARE LOOKING FOR

Individuals or families may often need guidance and psychosocial support to help cope with chronic, acute or terminal illnesses. Our Ambulatory Social Workers are an important link in this support team. These are the people who advise, educate, and counsel patients and their families. From providing crisis intervention, to referring patients to appropriate services, this job has it all.

WHAT YOU WILL DO
  • Collaborate with multidisciplinary teams to evaluate and assess patients’ medical and physical needs.
  • Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness.
  • Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
  • Identify environmental impediments to patient progress through interviews and review of patient records.
  • Assist patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources.
  • Assist in crisis intervention.
  • Document patients’ electronic medical records in a clear, concise, accurate, and timely manner.
WHAT IT TAKES TO SUCCEED
  • Manage conflict and tactfully handle complaints and difficult situations.
  • Customer service skills.
  • Competent when engaging with diversity.
  • Provide assistance to patients regarding the financial assistance and insurance processes.
  • Critically evaluate and analyze information in written materials.
  • Knowledge of the role of the Healthcare provider related to insurance processes.
  • Crises Intervention theory.
  • Knowledge of patient billing processes and means of paying for care.
  • Solve problems by analyzing information and using logic to address issues and problems.
  • Develop effective relationships with patients, hospital departments, and other external parties.
  • Effectively communicate verbally and in writing.
