Patient-first therapy care coordinators

WHO WE ARE LOOKING FOR

Provide professional guidance, mentorship and training to therapy staff. Provide program oversight and safety. Promote training and communication. These are just a few of the responsibilities our Therapy Care Coordinators handle. It’s a hands-on, fast-paced position in an environment that values compassionate, joyful service.

WHAT YOU WILL DO

  • Consult and advise manager on programs, projects, and activities of the assigned areas.
  • Assist patients/residents and families in securing services through community resources, which includes interpreting agency regulations for eligibility of such services.
  • Promote communication through positive interaction with patients, residents, families, physicians and staff.
  • Assist the manager in preparation for Department of Child Services (DCS) adolescent residential audits and HFAP accreditation surveys.
  • Assist the manager as community liaison including attendance at designated community events and committee meetings.

WHAT IT TAKES TO SUCCEED

  • Knowledge of professional regulation and facility/program accreditation requirements.
  • Understanding of scheduling procedures and health care delivery models.
  • Active listening skills.
  • Adapt and respond quickly to changing priorities.
  • Time management skills.
  • Strong communication skills, both verbal and written.  


WHAT WE'RE LOOKING FOR

 

1-3 Years Experience

License(s): Licensed Clinical Social Worker; Marriage and Family Therapist; Mental Health Counselor

Certification(s): Cardiopulmonary Resuscitation

Travel: Never or Rarely
