Patient-first rehabilitation coordinator


Coordinate care and provision of services to adult patients. Gather admission information for review. Help develop plans of care and goals. These are just a few of the responsibilities of our Rehab Coordinator. It’s a job that requires a special touch and the ability to establish good patient relationships.   • Gather admission information for review by the Medical Director and Clinical Manager for assessment purposes.
  • Document on referral charts the approval or denial. 
  • Input the 3 day admission FIM scores on the UDS Pro software for CMS.
  • Work with the rehabilitation team to establish patients’ goals and discharge planning needs. 
  • Plan of care and goals match the patient’s developmental age and emotional level. 
  • Submit 3 day admission FIMS and 24 hour discharge FIMS to UDS MR in a timely manner.
  • Market the services of the Rehab Unit to a broad range of referral sources including hospitals, home health agencies, physicians, nursing homes and community service providers. 
  • Prepare and deliver presentations to referral sources. 
  • Participate in marketing activities developed by the hospital.
  • Provide medical information to third party payers to assure coordination of benefits. 
  • Use financial information available to assist in making decisions regarding the provision of services. 
  • Facilitate timely and appropriate discharge planning. 
  • Arrange for needed equipment and services prior to patient transfer or discharge as prescribed by physicians and therapists, and in coordination with the patient’s payer source and financial means.
  • Provide patients and families/caregivers with initial information regarding the Rehab Unit and rehab process.  


  • Maintain excellent and ethical relationships with all referral sources and third party payers Utilize time productively.
  • Communicate effectively with patients at a level consistent with the patients’ developmental ages and emotional states. 
  • Communicate effectively with families, caregivers, significant others and members of the healthcare team to promote maximum benefit and coordination of care.



1-3 Years Experience

License(s): Exercise Physiologist Certified; Registered Nurse; Registered Respiratory Therapist

Certification(s): Basic Life Support Program

Travel: Never or Rarely
