Franciscan Physician Network is seeking an Internal Medicine Nurse Practitioner with 2 years of experience to meet the growing demand at our Heartland Internal Medicine in Camby, Indiana. 


Ideal candidate will have a focus on clinical excellence and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.

  • Schedule: Monday – Friday 8:00 a.m. – 5:00 p.m.
    • With one early morning and late night
  • Average goal volume: 18 - 20 patients per day after 91 days of employment
  • Weekend & Holiday Call: 1:4
  • Performs relevant patient assessments and physical examinations, including Medicare Wellness Exams
  • Coordinates, directs, and implements patient's plan of care. 
  • Orders, conducts, and interprets pertinent laboratory and diagnostic studies and procedures. 
  • Makes medical recommendations and/or referrals. 
    • Formulates, implements, and evaluates an outcome-based treatment plan. 
    • Collaborates with patients, families, other clinicians, and referral services to assist in the patient's recovery. 
    • Facilitates patient education relating to medical processes, medication, nutrition, and health promotion.
  • Maintains appropriate records detailing the patient's treatment plans and outcomes. 
  • Other duties as assigned.
