WHAT WE NEED

Caring, multi-tasking home healthcare professionals

WHO WE ARE LOOKING FOR

Sometimes the best healthcare is offered right in the patient’s home. Our home health intake clinicians go above and beyond to assist with planning and scheduling of patient care. They are also responsible for the intake of telephone referrals, case distribution, and related follow-up activities.

Are you able to deliver accurate and compassionate care? Do you have experience with home healthcare standards and regulations? Are you responsible and reliable? If so, we need your skills. 

WHAT YOU WILL DO
  • Assist billing and tracking non-paid claims, including researching information and documentation for these other insurance-related billing problems.
  • Investigate and determine initial patient benefits coverage for requested services, which includes Medicare, Medicaid, private insurance, HMO's, PPO's, grants, special contracts, charity, and fee structures.
  • Assist customers, patients, families, physicians, and clinicians via telephone with clinical issues, problems, and questions.
  • Work collaboratively with referral sources including hospital, ECF and ACO discharge planners and care coordinators to facilitate continuity of care and appropriate care.
  • Act as a liaison with pharmacy, medical supplies, DME and other vendors providing care to VNS patients.
  • Accept and process physician orders via telephone.
  • Communicate with all referral sources to gain the necessary information to facilitate continuity of care and appropriate care of patients and clients. 
  • Complete appropriate referral forms and enter information into the computer.
  • Develop and implement clinical review systems for all pay sources to include admissions, continued stay, and concurrent and retrospective reviews.
  • Document interventions taken and orders received as indicated.
  • Obtain managed care and insurance authorizations to determine the applicable coordination of benefits.
  • Schedule patient visits for field staff to ensure that the frequency of visits are compliant with the plan of care.
WHAT IT TAKES TO SUCCEED
  • Prior Home Healthcare experience.
  • Adapt to high volume and high stress areas.
  • Basic knowledge of computer software, hardware, and communication devices to perform tasks.
  • Customer service skills.
  • Data entry.
  • Knowledge of common diagnostic procedures.
  • Work independently with minimal supervision.
  • Handle multiple responsibilities simultaneously.
  • Understanding of patient billing processes and means of paying for care.
  • Quality improvement processes.
  • Develop effective relationships with patients, hospital departments, and other external parties.
  • Convey complex subject matter, simply.
  • Effectively communicate verbally and in writing
  • Solve problems by analyzing information and using logic to address issues and problems.
