That someone is a caring Patient Service Representative who works hand-in-hand with the patient care staff of the Hospice unit.   • Seek data necessary to complete admission record and labels.
  • Input data into admission record and computer system.
  • Update client’s record in computer system as needed.
  • Schedule client admission appointment and provides client information regarding appointment.
  • Reconcile registration process through computer system.
  • Prepare client chart prior to staff visit to include all pertinent documents and forms.
  • Maintain client record in appropriate functional order.
  • Perform computer functions e.g. for admit, transfer, discharge, status change, and monitoring for new orders.   
  • Audit consents for completion and accuracy.
  • Complete necessary forms and requisitions, enters into computer system.
  • Monitor hospice re-certifications and communicates information to the team.
  • File diagnostic reports and correspondences.
  • Copy and release medical information according to policy and HIPPA regulations.
  • Purge medical records for storage per department policy.
  • File client record in Medical Records room and keeps room organized.
  • Schedule client follow-up visit, treatment, labs, diagnostic tests and/or therapies according to physician orders.
  • Collect client co-payment as applicable.
  • Provide client directions for off-site services.
  • Track Room and Board payments, posting, and doing last day charge.
  • Greet incoming clients and performs check-in according to department procedure.
  • Assist in bed assignment process.
  • Assist with Room Readiness.
  • Introduce self and explains process.
  • Explain or direct patients to appropriate areas of the Hospice program.


  • Maintain current knowledge of all electronic programs utilized by the department.
  • Maintain confidentiality of client records and information.
  • Knowledge of medical terminology.
  • Strong communication skills, both verbal and written.
  • Experience with Microsoft Office.
