Just who is the RN Patient Placement Coordinator? A person who thrives in a fast-paced, busy environment.  A person who is able to efficiently coordinate the placement of patient admissions and transfers.   A person who can help assess levels of care for our patients. Are you an Registered Nurse ready for the challenge of identifying placement for patient admissions and transfers? Can you work as part of a medical support team to ensure best patient outcomes?  Are you efficient and organized? We need your skills.

WHAT YOU WILL DO

  • Identify and place all patients, including direct admits and transfers into the right bed, in an expedited manner, working to eliminate delays and obstacles. 
  • Collaborate with other departments as needed to prioritize timely bed placements.
  • Evaluate the appropriateness of admission. Appropriately plan for bed availability with information about scheduled surgery cases.
  • Incorporate knowledge of hospital and unit admission criteria, unit specialties, limitations and patient needs when determining proper patient placement.
  • Evaluate and negotiate appropriate patient placement, prioritizing where appropriate according to unit census, staffing situations and knowledge of acuity.
  • Coordinate with environmental services and patient care units to speed turnaround of beds.
  • Conduct daily bed meetings with clinical areas and intervention meetings when necessary.
  • Escalate discharges and challenges when necessary.
  • Address inappropriate admission criteria with admitting physicians.
  • Organize placement process to meet patient load influx and eliminates bed assignment delays.
  • Communicate with the Emergency Department staff frequently .
  • Check patients for isolation needs.
  • Assess, in collaboration with charge nurses and physicians, levels of care for patients in critical care unit, telemetry and step-down patients who can be moved to medical/surgical units if patients with higher acuity are admitted.  If no resolution, work with medical director and nursing director of those areas to resolve.

WHAT IT TAKES TO SUCCEED

  • Proficiency in Microsoft Office with an ability to adapt to and learn new computer software related to operations of bed management.
  • Excellent verbal, written communication skills, organizational skills and an ability to act on multiple tasks by prioritizing.
  • Follow established policy and procedures when performing job functions.
  • Sound judgment, problem solving ability and critical thinking in a fast paced environment.
  • Accountability for own actions, openness to change and a learning attitude toward problem solving.
  • Excellent customer services skills to provide exceptional patient care.
